Each report begins with a cover page that include a facility photo, facility name and address, as well as the date of inspection.

Every deficiency identified from the Facility Survey is included in the report. A single deficiency includes:

  • Category
  • Title of the element where the deficiency originated
  • Description of the finding
  • On-site finding
  • Recommendation and costing information
  • Federal and State codes (when applicable)
