DACTrak Reports Cover Page
Each report begins with a cover page that include a facility photo, facility name and address, as well as the date of inspection.
DACTrak Reports Deficiency Page
Every deficiency identified from the Facility Survey is included in the report. A single deficiency includes:
- Category
- Title of the element where the deficiency originated
- Description of the finding
- On-site finding
- Recommendation and costing information
- Federal and State codes (when applicable)
DACTrak Reports Final Page
The last page of the Accessibility Report includes:
- Facility photo
- Facility name and address
- Date of inspection
- Estimated facility total
